St. Joseph’s Primary Care Center – West

St. Joseph’s Primary Care Center – West formerly known as Westside Family Health Center provides health care for people of all ages in one convenient location. It has been a part of St. Joseph's Hospital Health Center since January 1994. The center provides primary health care for adult, pediatric, OB/GYN, and Behavioral Health Care services and offers 24-hour physician coverage, seven days a week.

About our health care team

Our staff consists of physicians, nurse practitioners, physician assistants, certified nurse midwives, nurses and support staff. We have an on-site nutritionist and social worker, as needed. In order to meet the needs of our large Hispanic population, we provide Spanish/English translation at the center. We also have a translator service to accommodate those who speak other languages.

Appointment hours

VISITS ARE BY APPOINTMENT. Please call 315-703-2600 in advance to make an appointment. Patient appointments are Monday through Friday: 8:00 a.m. to 5:00 p.m. with extended evening hours. To better care for our patients, we ask that you either keep your appointment or call in advance to reschedule. Sick appointments available daily.

Cancel Appointments

Please call the office at least 24 hours in advance. This will allow us to see other patients that need medical attention at that time.

If you need a prescription/medication refill

Please request routine prescriptions or refills when you have a visit with your doctor. If you do not have a scheduled doctor's visit, please call your pharmacy or the office three to five days in advance of when your prescription will run out. You will need:

  • The name of your prescription
  • The Dose
  • How often you take it
  • The number of pills you have left
  • The name and the phone number of your pharmacy
  • Your doctor's name

If you need hospitalization

If you or a member of your family (except children) must be hospitalized, you most likely will be admitted to St. Joseph's Hospital Health Center. Children will be admitted to Crouse or University hospitals.

Emergencies

If you have a life-threatening emergency, go to the nearest emergency department and identify yourself as a patient of the St. Joseph's Primary Care Center West. If your medical problem is not life threatening but requires care relatively soon, call the office at 315-703-2600. When the office is closed, your call will be forwarded to the on-call answering service. A physician is on call 24 hours a day.

Address or Telephone Number Changes

If you change your address or telephone number, notify our staff so that this information may be added to your record to keep it current.

Payment for services

We accept Medicare, Medicaid, PCAP and most insurances. Your participating insurance will be billed for services rendered. Co-payment is due at the time of service. If you have no insurance or need assistance with payment, a financial representative will assist you.

St. Joseph’s Hospital Primary Care Centers West, Pediatric West and Family Medicine have been awarded Level 3 recognition by the National Committee for Quality Assurance (NCQA). Recognition shows that we have the tools, systems and resources to provide our patients with the right care at the right time.

MyStJosephs

St. Joseph’s has a new electronic medical record, EPIC, that offers a patient portal called MyStJosephs. MyStJosephs offers patients the opportunity to communicate with providers, update their health information, and receive health reminders for upcoming recommended appointments and tests. Patients can also make online bill payments as well as review recent lab results, anytime from work, home, office, or on the road. www.mystjosephs.sjhsyr.org

New York State Department of Health HIV Primary Care Medicaid Program

St. Joseph’s Hospital Health Center participates in the New York State Department of Health HIV Primary Care Medicaid Program in order to reach out to patients to facilitate early treatment options. For more information please call 315-448-5480.
