Health Assessment



Employee Health Office Clearance

Before you begin your new role at St. Joseph’s Health, a pre-employment physical with our Employee Health Office is required. This helps us guarantee all of our employees are able to successfully perform their job duties.

Please be sure to schedule your pre-employment physical through our Health Office as soon as you have completed your drug screening. Health Office clearance must be completed prior to your first day of employment. Please call our Employee Health Office at 315-448-5581.

Where to go…

The Employee Health Office is located at 201 Prospect Avenue. You may park in the MOC (Medical Office Center) for your pre-employment physical. The entrance to the parking garage is on Union Ave. Please bring your ticket with you for validation.

What to bring…

When you visit Employee Health for your pre-employment assessment, please bring the following:

  • Pre-Placement Health Questionnaire and Assessment; please print and complete this form prior to your arrival. If you are not able to complete this form ahead of time, plan to arrive 15 minutes prior to your scheduled appointment to complete it. 
  • Driver’s License or other picture identification. 
  • Immunization Records, including documentation of: 
    • Prior PPD skin tests 
    • MMR (measles, mumps, rubella vaccine) 
    • Tetanus status 
    • Hepatitis B vaccine history (vaccine required for all employees working in patient care areas).

What to Expect…

During your exam in Employee Health, our staff will complete the following:
  • PPD Skin Test (TB skin test): Your Employee Health provider will provide details.
  • Fit Testing for TB Respirator Mask: For those with direct patient contact. 
  • Physical Examination: Completed to determine ability to perform the requirements and functions of your new role.
  • Immunization Analysis: An assessment of immunization status and the need for booster shots or titers. 
All offers of employment are contingent on being cleared by Employee Health based on the above listed criteria as well as the successful completion of the pre-employment drug screen.

What’s Next…

Immediately following your Employee Health Office visit, please report to the Human Resources office to have your picture taken for your ID badge. Human Resources is located in the office directly adjacent to the Employee Health Office.
