St. Joseph's Health Food Farmacy
Get the support you need to take back your health.
The Food Farmacy at St. Joseph’s Health is a grant-funded program for patients across the continuum at St. Joseph's Hospital, who have long-term physical or mental health conditions and don’t have access to affordable, nutritious food. The Food Farmacy offers eligible participants the food needed to live a healthier lifestyle, and direct access to a Community Health Worker who can assist in connecting participants with local programs and services such as SNAP, WIC, food pantries, housing, programs for seniors, etc.
To be entered into the program, you must:
• Be a patient of Primary Care, Outpatient Behavioral Health, or have been admitted through the Emergency Department and Inpatient Units
• Have a chronic condition (examples: diabetes, high blood pressure, congestive heart failure, mental illness, etc.)
• Face food insecurity
Once enrolled in the program, you will receive:
• Personalized nutritional counseling from a Registered Dietitian
• Access to care and local resources through the Community Health Worker
• Fresh and healthy food, including recipes, that consists of three days’ worth of food each week for a 15 week period
Participation in this program, including food, is free.
