Trinity Health Plan New York (HMO) is a Medicare Advantage organization with a Medicare contract. Enrollment in Trinity Health Plan New York depends on contract renewal. Benefits vary by county. Other pharmacies, physicians or providers are available in the plan’s network. Speak with a licensed sales agent during call center hours: April 1 to September 7, 8 a.m. to 8 p.m., Monday through Friday; September 8 to March 31, 8 a.m. to 8 p.m., seven days a week. A licensed sales agent will be present with information and applications. For accommodation of persons with special needs at meetings, call 800-964-4525 (TTY: 711). Not all 2027 plans have $0 Premium, Cash Back ($840.00 per calendar year, which is $70.00/month), comprehensive dental or Part D prescription drug coverage benefits.
