Pre-Employment Health Assessment

Applicants, congratulations on accepting a job offer from St. Joseph’s Health! As part of the hiring process, you will be required to have a pre-employment health assessment with St. Joseph’s Health Employee Health Office before your orientation date.

Please contact the Employee Health Office at (315) 448-5581 (press option 1) to arrange your appointment.  Please plan on arriving 15 minutes before your appointment time to fill out the necessary paperwork.  Allow up to 2 hours for completion of your appointment.

Important things to note:

  • If you park in the parking garage, please bring your parking ticket to your appointment and we will validate your parking.  The address for the garage is 104 Union Avenue, Syracuse, NY.
  • Please bring with you any immunization records you have or documentation of positive titers for Measles, Mumps, Rubella, Varicella, and Hepatitis B (if you have completed the vaccine series) and a copy of a PPD (TB skin test) or lab result for QuantiFERON Gold for TB screening if you have had one completed in the last 12 months.  Also, please bring documentation of your COVID Vaccine as well as a copy of your Flu Vaccine if you have received it for the current Flu season.  If you don’t have your immunization records, blood work to check titers will be done at the lab in the Medical Office Building on the day of your appointment.
  • Print and fill out the attached Pre-placement Health Questionnaire and Assessment and bring them to your appointment.
  • After your appointment, you will go to Human Resources (located upstairs from the Employee Health Office in the same building) to have your picture taken for your ID Badge and finalize paperwork with the Onboarding Coordinators.

DIRECTIONS TO EMPLOYEE HEALTH OFFICE FROM GARAGE

  • Take the elevator to the 4th floor of the parking garage.
  • Exit to your left (parking circle) and walk up the street to the corner.
  • Cross at the pedestrian crosswalk and turn to your left (walk away from the hospital)
  • The Health Office is located at 201 Prospect Ave. (white brick building)
  • Follow the signs to the courtyard at the back of the building 
  • Press the doorbell for entry into the building.
  • The Health Office will be to your left, just past the elevator.
  • If you get lost, contact the Employee Health Office at 448-5581.
