St. Joseph’s Health Food Farmacy

Get the support you need to control your diabetes.

The Food Farmacy at St. Joseph’s Health is a grant-funded program for patients of Primary Care Centers Main and West, as well as Outpatient Behavioral Health, who have chronic conditions and don’t have access to affordable, nutritious food. The Food Farmacy offers eligible participants the food needed to live a healthier lifestyle, and direct access to a Community Health Worker who can assist in connecting participants with local programs and services such as SNAP, WIC, food pantries, housing, programs for seniors, etc.

In order to be entered into the program, one must: 

  • Be a patient of Primary Care Main, Primary Care West, or Outpatient Behavioral Health, including the PROS Program
  • Have a chronic condition (examples: diabetes, high blood pressure, congestive heart failure, mental illness, etc.)
  • Be food insecure – participants will be screened by the Community Health Worker

Once enrolled in the program, patients receive:

  • Nutritional counseling from a registered dietitian.
  • Access to care and local resources through the Community Health Worker.
  • Fresh and healthy food, including recipes, that consists of three days’ worth of food each week for a 10 week period

Participation in this program, including food, is free.
