We've created a screening questionnaire based on CDC guidelines to help determine the most appropriate level of care for you. All coronavirus (COVID-19) screenings are free of charge.

  1. Complete an online health interview: 5-15 minutes
    1. Answer a series of questions about how you are feeling and the symptoms you are experiencing—just as you would during an in-office visit.
  2. You will be given guidance on next steps for you to take based on your answers, including recommendations to contact a physician for further evaluation.
    1. If you require further evaluation, we have excellent, board-certified providers who are accepting new patients.
